Breastfeeding While Sick: What You Actually Need to Know

Few things bring in more calls, texts, and emails than this: “What can I take when I'm sick?” A cold. The flu. An upcoming surgery. A CT scan with contrast. And almost always, somewhere in that conversation, comes the phrase "just pump and dump."


We need to talk about that phrase.


"Pump and Dump" Isn't the Automatic Answer

Pumping milk out and pouring it down the sink is heartbreaking, and far too often, it's simply not necessary. It's also risky in its own way: if a mom is told a medication is safe and it isn't, or she's told to dump when she didn't need to, her supply can take a real hit. Before reaching for pump-and-dump as the default, it's worth asking a more basic question: what does this illness actually need to treat it? Sometimes the honest answer is rest, hydration, and a nap. Sometimes it's a walk outside, bare feet in the grass, time in the sunshine, or a swim. Vitamin D matters here, too. It plays a real role in hormone regulation and immune function.


For the Common Cold

Old remedies often work best. A neti pot with saline to rinse the sinuses. A cup of tea with lemon and honey to soothe the throat. When antibiotics or steroids are actually needed, there are plenty of options that are safe while breastfeeding. No weaning, no pumping and dumping required.


The real threat to supply with a cold isn't the illness; it's certain remedies. Peppermint tea can dry up milk. So can decongestants like Sudafed or phenylephrine, which is exactly why lactation consultants sometimes recommend them to moms who are ready to wean. If that's not your goal, steer clear. Cough syrup without a decongestant is generally fine. Cough drops without menthol are usually fine.


Stomach Bugs and Hydration

Imodium is considered safe. Pepto-Bismol is not, even though it's over-the-counter and feels harmless. With any GI illness, hydration is usually what determines what happens to supply, not the illness itself. It's easy to forget to drink, or to be vomiting and unable to keep fluids down. Antibiotics, Thrush, and Yeast

Most antibiotics that are safe for a baby are safe for a breastfeeding mom to take. That said, antibiotics can bring side effects (GI upset, gas, bloating) for mom and baby alike, and some raise the risk of yeast infections. A yeast infection on the breast is no small thing, and it often needs real treatment: a prescribed cream for the nipples, or a prescription for baby's mouth if thrush passed from baby to mom (or the reverse).


Prevention matters just as much as treatment. Sterilize anything that touches the breast or baby's mouth: pump flanges, nipple shields, and bottle nipples. Adding white vinegar to the wash changes the pH of fabric and discourages yeast growth on bras, nursing pads, and clothing. If you have thrush, the milk you pump is still fine to use. Please don't dump it.


Breastfeeding Sick: How Your Body Protects Your Baby

Breastmilk is a living substance, constantly adapting to your baby's needs. When your body is fighting an infection, it's producing antibodies and shifting the composition of your milk to help protect your baby, who's likely already been exposed to whatever you have anyway. Breastfeeding sick or continuing to pump through illness (when medically appropriate) isn't a risk to your baby in most cases. It's their best line of protection.


Migraines, Combinations, and Where to Get Real Answers

Medication safety gets more complicated with migraines and multi-drug regimens. A single medication might be fine on its own but riskier in combination with others. This is exactly the kind of question worth a real answer, not a guess. Surgery, Anesthesia, and Imaging

There's a lot of controversy around surgery and breastfeeding, but the Academy of Breastfeeding Medicine has published protocols on anesthesia safety, and in most cases, general anesthesia does not require pumping and dumping. Know which medications will be used, tell your care team you're breastfeeding, and go in informed.


CT scans with contrast are also generally considered safe for breastfeeding, even though many facilities default to instructing 48–72 hours of pumping and dumping. The research doesn't support that being necessary in most cases, but read the documentation for your specific situation and make an informed decision with your provider. When It's Not Safe

To be clear, there are real exceptions: chemotherapy, radioactive dye, and certain psychiatric medications are not compatible with breastfeeding. The goal isn't to say everything is always fine. It's to know the difference between what genuinely isn't safe and what's been assumed unsafe out of habit or caution.


The Bottom Line

Ask the questions. Get real answers. Protect your supply. Don't let an illness derail a breastfeeding journey that doesn't need to end. Sustainability includes staying informed when you're sick, not just when things are going smoothly.
